We are keen to learn about your school and how you currently organise your uniform and accessories.  For your time in completing this questionnaire, we will send you a FREE Blue Max fleece (adult or child) - please indicate at the end of the form the size and colour you prefer.  We regret this offer is open to bone-fide schools based in the UK only.

It may be helpful to read about our Schemes for Schools before proceeding.
